Admissions/Discharge Nurse

Job Title:                   Admissions/Discharge Nurse

                                    

Hours:                       Monday - Friday 4 - 8 pm

 

Department:              6240 Intermediate

                                         

Job Function:            A member of the professional staff who has the responsibility of obtaining medication and medical history information from patients prior to admissions through preadmission phone calls.  Responsible for making post discharge phone calls to assess the patient’s current status and to get an understanding of their treatment plan.

 

Job Qualifications:         

·         Education: Graduate of an accredited school of nursing.

·         License: Registered Nurse with current Kansas licensure.

·         Previous Experience:  2 years in an acute cardiovascular setting preferred.

·         BLS certification required.

·         Interpersonal Skills:  highly self-motivated individual who demonstrates and promotes confidence, credibility, adaptability, initiative, and problem solving.  Requires strong communication skills.

·         Familiar with electronic health records systems.

                                         

Job Duties and Responsibilities:    

·         Make preadmission phone calls

·         Obtain patient’s home medication information

·         Enter information into the hospital’s EHR.

·         Review admission process with the patient/family and answer questions.

·         Make post discharge phone calls to patient/family.

·         Ensure patient/family comprehend the discharge instructions and medication regimen.

·         Other duties as assigned
